Serious Injuries Prompt Recall of Mattel’s Polly Pocket Magnetic Play Sets

Name of Products: Polly Pocket dolls and accessories with magnets

Units: About 2.4 million play sets (an additional 2 million play sets were sold worldwide)

Importer: Mattel Inc., of El Segundo, Calif.

Hazard: Tiny magnets inside the dolls and accessories can fall out undetected by parents and caregivers. The magnets can be swallowed, aspirated by young children or placed by a child in their nose or ears. When more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforation, infection or blockage, which can be fatal. Aspiration to the lungs requires immediate surgery. Magnets placed in the nose or ears can cause swelling and be difficult to remove.

Incidents/Injuries: CPSC is aware of 170 reports of the small magnets coming out of these recalled toys. There were three reports of serious injuries to children who swallowed more than one magnet. All three suffered intestinal perforations that required surgery. A 2-year-old child was hospitalized for seven days and a 7-year-old child was hospitalized for 12 days. An 8-year-old child was also hospitalized.

Description: The recalled Polly Pocket play sets contain plastic dolls and accessories that have small magnets. The magnets measure 1/8 inch in diameter and are imbedded in the hands and feet of some dolls, and in the plastic clothing, hair pieces and other accessories to help the pieces attach to the doll or to the doll’s house. The model number is printed on the bottom of the largest pieces on some of the play sets. Contact Mattel if you cannot find a model number on your product to determine if it is part of the recall. Polly Pocket magnetic play sets currently on store shelves are not included in this recall.

Hazard: The handles on the puzzle pieces can come off, posing a choking
hazard to young children. Also, the tips of the nails on the inside
shelf of the puzzle table could be exposed, posing a laceration or
puncture hazard.

Incidents/Injuries: Target has received two reports of the pegs coming
off of the puzzle pieces. In one of these reports, a child was found to
be mouthing the pegs. No other injuries were reported relating to these
hazards.

Description: This recall involves a "Play Wonder" brand puzzle table
that contains three nine-piece wooden puzzles inside the table. They are
light blue with paw prints painted across the top of the table. The
table is about 13-inches high when assembled and has green sides. The
"Play Wonder" logo is located in the front lower right corner of the
packaging. Item number 204/12/0162 is located in the front upper right
corner of the packaging.

Hershey products pulled off Canadian shelves

Certain candy bars as well as chocolate chips for baking are among the products Hershey Canada is pulling off store shelves because of possible salmonella contamination.

Hershey Canada Inc. and the Canadian Food Inspection Agency released a list on Sunday of 25 recalled products.

Any foods tainted with salmonella may not look or smell spoiled, but the bacteria can cause symptoms such as high fever, severe headache, vomiting, nausea, abdominal pain and diarrhea.

Hershey's recalled products can be identified by date codes - ranging from 6417 to 6455 - found on the back of each package. They include:

- Hershey Chipits Milk Chocolate Chips - 270 g.

- Hershey Creamy Milk Chocolate With Almonds - 43 g.

- Hershey Creamy Milk Chocolate - 45 g.

- Oh Henry! - 62.5 g; 62.5g/4 bars; 145 g.

- Oh Henry! Bites - 130 g.

- Oh Henry! Peanut Butter - 60 g.

No products produced for Halloween or Christmas are affected, said the company.

The contamination at the company's plant in Smiths Falls, Ont., is believed to be the result of an ingredient shipped in from an outside supplier. The plant was closed Thursday evening after the salmonella was discovered.

"It’s a minor ingredient used in chocolate making. That's all I can share at this time," Hershey spokesperson Stephanie Moritz told CBC Newsworld on Monday.

It's not clear how much of the affected chocolate, produced between Oct. 15 and Nov. 10, may have reached store shelves, said officials.

"There was a limited amount of product that was actually at retail level," said the CFIA's Garfield Balsom. "The majority of the product is being contained at distribution and warehouse levels."

Moritz said the contamination involved a limited number of products.

"Most of the product is already in our control or being retrieved from customers," she said.

Balsom wouldn't speculate about what may have caused the contamination, saying an investigation is ongoing.

"The salmonella could be introduced through various sources. The investigation needs to be focused and detailed in order for us to determine where this was introduced."

There have been no reported illnesses associated with the consumption of the candy or chocolate chips on the recall list, the company said.

Moritz said the discovery shows the company's quality and safety checks work.

11 million bottles of store-brand acetaminophen 500-milligram caplets.

Nov. 9, 2006 -- The FDA today announced a recall of about 11 million bottles of store-brand acetaminophen 500-milligram caplets.

Those caplets are over-the-counter drugs for pain relief and fever reduction. They're sold by about 130 companies, including Wal-Mart, CVS, and other drug stores, grocery stores, and wholesalers.

The recall is due to "small metal fragments found in a small number of these caplets," states an FDA news release.

The fragments range in size from "microdots" to 8-millimeter pieces of wire, according to the FDA.

The FDA and the caplets' maker, Perrigo Company of Allegan, Mich., are investigating how those metal fragments got into the caplets.

So far, no illnesses, injuries, or consumer complaints have been reported to the FDA or Perrigo, according to the FDA.

The FDA "believes the probability of serious adverse health consequences is remote," the FDA's Jason Brodsky told reporters in a teleconference.

Brodsky is the FDA's assistant commissioner for external relations.

"However, if a consumer were to swallow an affected caplet, it could result in minor stomach discomfort and/or possible cuts to the mouth or throat," Brodsky says.

Advice for Consumers

For a full list of the companies that sell store-brand 500 milligram acetaminophen made by Perrigo, visit the FDA's web site at www.fda.gov/oc/po/firmrecalls/perrigo/perrigocustlist.html.

The FDA's web site also lists the recalled batches of the drug. That list is posted at www.fda.gov/oc/po/firmrecalls/perrigo/perrigobatchlist.html.

The recalled drugs have expiration dates ranging from September 2006 through August 2009.

The FDA advises consumers who believe they have the recalled acetaminophen to "discontinue use immediately" and call Perrigo's consumer affairs department at (877) 546-0454 for further instructions.

People who think they may have been harmed by the affected caplets should talk to their doctors, says Brodsky.

"We believe, at present, that this is a relatively small fraction of the total available acetaminophen on the market," the FDA's Douglas Throckmorton, MD, told reporters in the teleconference. Throckmorton is the deputy director of the FDA's Center for Drug Evaluation and Research.

Any adverse reactions to the products should be reported to the FDA's MedWatch program, which can be reached by phone at (800) FDA-1088, by fax at (800) FDA-0178, or on the MedWatch web site at www.fda.gov/medwatch.

Adverse reactions should also be reported by phone to Perrigo's consumer affairs department at (877) 546-0454, says the FDA.

Investigation Under Way

According to the FDA, Perrigo told the FDA about the problem on Nov. 1.

Perrigo discovered, through its own regulatory quality-control procedures, that their tableting equipment was wearing down prematurely, says the FDA.

The FDA says Perrigo told the FDA that 70 million caplets went through a metal detector, resulting in the discovery of about 200 caplets containing metal fragments ranging from "microdots" to portions of wire 8 millimeters long.

Perrigo is working with the FDA on correcting the problem.

In a Perrigo news release, the company says it bought raw materials for the caplets from a third-party supplier. Perrigo didn't name that supplier.

John Hendrickson, Perrigo's executive vice president and general manager of Perrigo Consumer Health Care, commented on the recall in Perrigo's news release.

"Our quality control systems noted trace amounts of metal particulate in a very small number of these caplet products," Hendrickson says.

"Although the frequency of occurrence is very low, the probability of health risk is remote, and there have been no reports of injuries or illness related to this incident, we are taking this measure to maintain the highest possible product quality standards for our retail customers and their store brand consumers."

It consists of a soft, stuffed body with a rattle component inside the body and two smooth wooden rings attached to loops at the end of each arm. The product is available in four different styles: Rumba Lion Activity Toy with style number 58553, Gazoo Giraffe Activity Toy with style number 58554; Wango Horse Activity Toy with style number 58555, and Tippy Cow Activity Toy with style number 58556. “Woodles™” and the style number are located on the sewn-in label.
