2 vessel cord-single umbilical artery-- WHAT IS IT??

This is something id like people to be aware of as i had never heard of it until my baby was diagnosed with SUA.

When i went for my 19week abnormalty scan everything seemed to be fine. Then the sonographer had realised there was a problem with the umbilical cord. In a normal Umbilical cord there is suppose to be 3 vessels,These vessels are made up of 2 arterys and 1 vein. Even tho only 1 artery is needed to sustain a normal healthy pregnancy, it still raises concern when the other artery is'nt present. The arterys are there to carry the babies waste through to the placenta and then back to the mum to dispose of. The vein is to carry the blood, nutrients and oxygen to the baby.

So what is SUA?? Sua is when there is only 1artery and 1vein present instead of 2arterys and 1 vein. This just means that when the umbilcal cord was developing the 2nd artery didnt appear or only grew small and isnt big enough to do the job.

Is SUA commom?? No not really, It only occurs in 1 in 100 live births and happens twice as much in Caucasion women then it would Asian and African American women. The male-to-female ratio is 0.85:1.

  • When are the chances increased of having SUA??
  • Female fetal sex
  • Maternal age over 40years
  • 3 previous children
  • Multiple fetuses
  •  

    The only affect that SUA will have on your pregnancy is having a baby at a low birth weight  <2500g, ive been told to expect nothing over 6pounds and that most women whos babies cord is affected with SUA deliver their babies early, around 37weeks. Also you will be closely monitored from 30weeks to make sure your baby is putting on weight and so they can keep track of how much weight is being put on.

    So what affect will SUA have on my baby?? One third of babies will suffer some sort of anomaly, half will suffer from structural heart abnormalities,just over a third had some gastrointestinal tract abnormality, while one fourth will suffer from some malformation of the central nervous system. While Two thirds of babies will have no problem at all besides a low birth weight.

    Some things that your sonographer will check for are cleft lip, spina bifida, ventral wall defects, and marginal and velamentous insertion of the cord. My sonographer checked for all of this and everyhting is fine and growing fine. The heart was also checked and the 4chambers are present and the heart looks normal and healthy. I also had the babies kidneys and bladder checked and all seem to be working correctly without any difficulty so i have been told not to be concerned, and that my baby will be one of two thirds who come out with no problem.

    Like any Mum/Mum2b would, i was still worried and couldnt stop thinking if my baby was ok. I brought this up with my midwife and she reassured me that everything is fine and everything will be, the only thing they are concerned about is his weight. While he was measuring to be 19weeks and that i was, babies tend to slow down a little in the last trimester and just store fat which they think he will not store much of..That is why they are thinking he will be a 5pound baby. They said they wouldnt expect much bigger off me anyway considering how tiny i am. I was only 5pound 9 full term and iv'e grown pretty talla and im healthy, so im not to worried if my baby is small, ill fatten him up.

    So the most common problems associated with SUA are the kidneys,bladder and heart, which all of these can be checked for while pregnant via Ultrasound.

    I'm 29weeks pregnant at the moment, and everything is fine.My belly is measuring correctly, bubs moves ALOT, he is a very powerful and active baby, he never seems to stop bashing up my insides lol..I have an ultrasound in 3weeks so they can get a rough idea on how much he weighs and if everything still looks fine there will be no more testing done. When he is born they will be doing a quick Ultrasound of his kidneys and they will take away the cord for testing.

    I would just like to end this by saying, if you are told there is only a SUA present dont get worried. Ask questions and im sure everything will be answered for you. Yes some babies will be affected and you need to prepare yourself for that if that may happen and like i said they can test everything and you will be told exactly what your most at risk of having problems with if anything. Im just so thankful that my baby is a lucky one who hasnt been affected, this is also because I dont relate to any of the increased chances such as my baby being a twin or me being over 40yrs old.

    Hi Dannii!  Likewise, I didn't hear about this until I had my u/s done with no2.  It took a long time and she wouldn't answer my questions - talk about freaking me out!  Anyway, she also mentioned a higher risk of club-foot, but, again they could find no evidence of any malformation or growth issues.  I was concerned as my first was born at 37w, weighing only 2.4kg (5 1/2lb) due to pre-eclampsia and I was hoping not to have another small baby.  When I mentioned the concern of a smaller baby my obstetrician told me he didn't know where I'd heard that and he had never heard it?  So, I am glad I am not the only one who read this somewhere - made me feel stupid.... Anyway, Bella was born at 40weeks exactly and weighed in at 3.3kg (about 7 1/2lb) with no health issues at all.

    I am so glad to hear that your baby looks like being unaffected by this. I hadnt heard of this til one of my friends had it while I was carrying DD2. She was also on her second baby and had such a horrible time with her DS1 that she really deserved a straightforward 2nd pregnancy, so she went though all sorts of agonies until they told her, like you, that he looked ok. She was already booked in for an elective C-sec at 38 weeks, but he was gaining weight and looking so comfortable by then that they persuaded her to wait another week. I forget how big he was by the time he was born, but I seem to remember he was pretty normal (but then anything would have looked normal beside his 10 week prem brother!) and now he towers over my DD2, even though she's not actually small! Sure your DS will do the same. Im sure your Article will be comforting to anyone who's just been diagnosed!

    Thanks for your comment Cath..No not many people have heard of this, my mum never even knew about it. Im just glad they have given me the all ok but be prepared for a small baby.

    You say you fit all critea- They would have checked the umbilical cord during your u/s and seen that both bubs have 3vessels so they wouldnt have brought it up.

    yeah i asked alot because i was so worried..they spent that much time going over everthing in the u/s that they were able to tell me it will all be good and not to stress. He can see that everything works and looks normal.They will still look at his kidneys when his born, thats the only part they cant get a very gud look at through u/s, the u/s needs to be done on him.. But both kidneys look to be the same size and work fine.

    Hey Danni,

    Not to scare you but...I had a single cord as well. What I did not know until afterwards was that by having a single cord your chance of having a child with a congenitial heart defect are MUCH greater, much greater. Specifically a VSD, Ventricular Septal Defect. The defect my son had.

    At your birth hospital, even if your baby looks fine they NEED to scan the heart, liver an kidneys. All the ensure everything went fine.

    ALL of my ultrasounds were fine as well. I had TONS of them because of  the 2 vessel cord and I had gestatinal diabestes. So I had at least 15 ultrasounds.

    A clear ultrasound does NOT in any way mean your child is clear form heart defects. OUS were ALL clear like I said. Congenitial defects like a VSD and an ASD are not caught on regl U/S. If they let you, you can request a fetal Echo. it will be done in a ped. cardiologist office. As a matter of fact IF I get pregg. again. I will go to the same office my son goes to for carefor his heart fo rmy fetal echo.

    After I went back fo rmy last appt. with my OBGYN after giving birth Jedd already had  his NG feeding tube. My OBGYN asked what was wrong (very nicely) I told him. He looked up all of my U/S pic. on the computer and sure enough, he could not even detect the VSD heart defect in ANY of them.

    If you goole the owrd, Single Umbilical cord and Congenitial heart defects. all together in one search you will pull up tons of articles aobut this and that it is true. I know of one other Mother who also had the single cord and he son had a VSD as well.

    I know earlier you sadi that you would like to leave right after the birth but....IMHO.....now that I knwo you have the 2 vessel card, then I woudl stay and make sure they run all the test possible to make sure the baby is fine.

    I hope it was okay to put this here....Let me know what the dr. says abou this. I would bring it up to him. The chances are VERY MUCH greater.

    Like I said they told  me and the other Mother I know that our babies were ifne via U/S and they were wrong. The only TRUE way to check fetal hearts is viw a fetal echo.

    We have already had a big talk with the doctors and covered everything. At my 19 week scan they spent just 40 minutes on the heart.He said he cant see no problems at all and the heart is beating fine.All four chambers were the the right size.His heart has always been at a good rate also and has been able to find since 5weeks. The heart defects affect affect all most half of the one third..Thats why i have been scanned so carfeully.

    The only thing they may be concerend about is his kidneys, but they said not to worry. He will definetly be a small bub tho. So i just dont worry now, they have assured me that thigs will be fine.

    Not to be persisitant but I would still get things checked after he is born. They told me the same exact thing.....and all was not fine. There ar esimple tests they can run after he is born and that will assure that all is well. U/S can't catch them. Jedd's should have been easy to catch IF it was possible to catch them via U/S. Hi shole was aobu t 1/2 in big about 7-8mm. It was HUGE.

    The reason I say they can't be detected by U/S is....the chambers of the heart are not fully closed while in utero. There is some opening, not much but some. After the baby is born the cheart chambers, obviously, close up to the proper way. So that is why U/S are not the most efffective way to catch a CHD.

    I am sorry if it sounds like I am bugging you aobu this but.....more children die from CHD than any other birth defect. If I can spare one Mother from the way our first 5 weeks went I would gladly do it. Jed was very very fussy baby to begin with and hardly ever ate anything. He just wanted to sleep all the time. It was becasue his body had NO energy to do anything else.

    One last thing and I'll stop...LOL I promise

    The advice I gave is what my son's cardiologist gave us on his his visit when we got our life changing news. He told us that I should have had a fetal echo done in his office. Since we have one child with a CHD I WILL have one next time becasue SUA or not, our chances are now upped because we have previous history.

    BTW all the Nurses and pediatricians in my birth hospital MISSED Jedd's heart condition with a stethiscope as well. Our home ped. missed it as well, that is becasue without a fetal echo a VSD or an ASD are not caugHt until a child is around 6 weeks old. Jedd's was caught at 40 days old by the ped. So our ped. actually caught it early.

    By that point his hole was soo big she could hear it in the front and the back as well as feel it from the outside with our fingers.

    Hi,

    The 20th of may of i have another scan.I'll be 32weeks then.The doctors had taken alot of testing and measuring of bubs heart which were all looked over carefully at my last app..I had the doctor come and talk to me about whats going on.The heart, kidneys and bladder all seem fine also the brain. At my 32 weeks scan they will check everything again but they are more concerned with his weight..

    They are very happy with his heart, and kidneys are both the same size and growing normally but they will check these things as soon as his born.I will ask the doctor about the heart condition and if i should have further testing, yer he had told me its all ok, but i will ask again, because im now becoming concered.

    I really don't mean to scare you...sorry, but it is always better to eer on the side of caution, KWIM???

    At least your OB knew about the connection, mine didn't so that is great. I know here in the US I had to sign a waiver many times saying I understood that U/S could not catch any birth defects. That way the OB's can't be held responsible for any unknown defects the chid is born with, like Jedd's heart condition. I think I signed quite a few of them...LOL I guess they kept loosing them??? LOL

    You know, it' no tlike gettign the fetal echo is going to change anything, KWIM?? I would jsut make sure that he gets scanned after he is born.

    It sounds like your OB has it together. Will you be delived by  your mid wife??

    Yes because they are so sure evrything is fine they are treating it like a normal pregnancy.So like anyone else i go into the hospital when i get the contractions or when my waters break.I have to get in there asap after my waters break tho cos i have Strep b, so they need to give me the meds.Also they are so sure nothing is wrong and  25% of babies are born with 1 or more problems where the other 75% of babies born have no defects.

    This is what i just found on the net:

    It is likely that you will receive a more thorough ultrasound scan in order to detect any abnormalities.Since ultrasound scans are very good at picking up abnormalities, if you have a normal ultrasound, then it is likely that your baby will be born without any congenital or chromosomal abnormalities.

    I had that more thourogh ultrasound and i got the a ok.

    So yeah normal delivery, he may come early tho, they say 37weeks.

    They induced me and Jedd was actually accidentally born at 38 weeks, he was 5lbs 15 ox. VERY small. What more funny is that I had Group B strep too...LOL That's too weird.

    Jedd must have been in the 25% because he had/has 2 congnitial birth defects. He had the heart VSD and he has a hypospadias. That will be repaired here next week, May 2nd. It a 3 hour surgery and at least 2 hours in the PACU with a possible overnight...blah!!!!!

    Hey,

    I hope everything goes well with Jedds surgery..

    Both my sisters and I were 5 pound 9 babies full term.We had nothing worng with us at all and have all grown healthy. My midwife said she wouldnt expect anything bigger then 6pound from me even if there wasnt this problem. Im very small, and my belly seems small but im measuring fine..Bubs is a very active little man, he just kicks all day long lol.

    Once again i hope Jedds surgery goes well, ill be thinking of you's.
