Have you discussed the problem with the day care? If you ask the person in charge of the room your son is in, she will be able to help. Explain to her the strategies that the speech therapist has recommended and ask for these to followed through out the day. Day care staff are normally happy to co-operate with you and this will reinforce the need to communicate to your son. Often children just find it easier to gesture rather than think about what they want and use the appropriate words.
My son required speech therapy and we found he improved dramatically when everyone around expected him to try to say his words clearly. We played dumb a lot of the time and pretended we weren't sure what he wanted. We asked him questions until he came up with a clear verbal response. We can't shut him up now. lol
Good luck, it sounds like you are on the right track but these things take time.
