poor baby.
20 minutes is long enough for a large percentage of the drug to be aborbed, so you will need to wait 4-6 hours before giving it again. be sure not to give any more than 4 doses in 24 hours. as tylenol is paracetamol based, you may give some nurofen as well, in between doses of paracetamol, or after a vomit, as these are 2 different drugs but have the same anti fever effect.
give him small amounts of fluid frequently. start with 2ml per kg every 20 minutes (this is what we call a trial of fluids). ie. if your son weighs 8kg, then offer 16ml every 20 minutes (use a very weak cordial- not strong, as the sugar will draw out fluid from his body, or you could buy some hydralyte from the chemist). if he tolerates this then gradually increase the volume. if he does not tolerate this then he may need to go to your local emergency dept or dr for assessment, particularly if he starts diarrhoea as well.
keep an eye on his hydration, if you are worried that he is having less wet nappies, he has no tears when he cries, if his mouth and lips look dry or if his eyes look sunken then he needs to be seen by a dr.

We have been told the same thing.
Can you call the ped and see if you can rotate the tylenol wiht Motrin??? Ours let us. We do tyelnol and 4 hours later do Motrin when Jedd has a fever.
Also when Jedd's got really high, they allowed me to care for him at home (Jedd has medical issues so I get ALOT of leeway to keep him at home during illness that would normally be sent toe he ER for). They gave me a higher than the bottles recommendation of the doese for both tylenol and motrin and also allowed me to roate on a 3 hour basis.
I, however, would NEVER do this on my own, CALL the PED.!!!!!
Don't get mad if they don't allow you to do this, for use like I said, I get ALOT of leeway with Jedd. And have ALOT of experience delaing with issues at home.
More than likely, they will send you to the ER for an eval after hours. It's jsut that for us, the ER is the WORSE place to send Jedd and his PEd and all the other Peds in the practice know this so we avoid it at all costs.

Durign sicknes was the ONE time I was extrmely thankful fo rJEdd's feeding tube. I could use it like a makeshift I.V. for hydartion all day nd all night....*YAY*
